(023) Systematic evaluation and meta-analysis on the treatment of moderate and severe cancer pain in cancer patients with traditional chinese medicine combined with three-step analgesic method

Background: The conventional three-step analgesic therapy for cancer pain may lead to drug resistance or even addiction after long-term use. Therefore, traditional Chinese medicine combined with three-step analgesic therapy has gradually become a new treatment method.

Objectives: Systematically evaluate the clinical efficacy and safety of traditional Chinese medicine combined with three-step analgesics in the treatment of moderate and severe cancer pain.

Methods: 1.The randomized controlled trials(RCTs) of traditional Chinese medicine in treating moderate to severe cancer pain were retrieved from the China Knowledge Network, Wanfang database, VIP database, PubMed, and Web of Science, and the retrieval time was set from January 1, 2005 to February 1, 2023.
2.According to certain inclusion and exclusion criteria, documents that meet the criteria will be included in the analysis.
3.The final included documents are evaluated according to the bias risk tool of Cochrane System Evaluation Manual.
4.The mainly data extracted in the literature are the total effective rate of cancer pain relief, the improvement rate of quality of life (KPS), and the incidence of adverse reactions (constipation, nausea, vomiting, dizziness, lethargy).
5.Meta analysis is performed through Revman 5.3 software.

Results: A total of 33 articles were included and 2545 patients were included. The combination of traditional Chinese medicine and three-step analgesic therapy for moderate and severe cancer pain can improve the remission rate of cancer pain [RR=1.29, 95% CI (1.22-1.36), P< 0.0001], improve the improvement rate of KPS score of quality of life [RR=1.49, 95% CI (1.19-1.87), P< 0.0001], and reduce the incidence of adverse effects such as nausea [RR=0.45, 95% CI (0.32-0.63), P< 0.0001], vomiting [RR=0.46, 95% CI (0.30-0.70), P=0.0003] The incidence of adverse reactions such as constipation [RR=0.44, 95% CI (0.33-0.59), P< 0.0001], dizziness [RR=0.49, 95% CI (0.33-0.73), P=0.0004], lethargy [RR=0.41, 95% CI (0.27-0.63), P< 0.0001].

Conclusions: Traditional Chinese medicine combined with three-step analgesic therapy for moderate and severe cancer pain has good efficacy and safety, and can improve the quality of life of patients.
